How to Apply

********NOTE: The application period to enroll in the Doctor of Physical Therapy Program for Fall 2008 has passed.
For Fall 2009 the physical therapy program is planning to begin using the national on-line application process, PT-CAS.  The application deadline will move up significantly to as early as November 2008.   Please check back frequently to stay up to date with the application process.*****************

 

Doctor of Physical Therapy (PT) Program Supplemental Application Checklist

All applicants to the PT program must include the following supporting documentation to their college application. Completion of the prerequisites with the minimum requirements does not guarantee an interview or admission to the professional program. Incomplete applications will not be considered for admission.
 
Application Checklist:
 
The following must be submitted by every applicant:
  • Completed College Application 
  • All applicants must complete the WSU graduate application: www.gradadmissions.wayne.edu
  • succesfully complete the admission requirements
  • Personal resume outlining your community or vocational activities, honors, employment, extracurricular and volunteer activities.
  • Transcripts from all colleges/universities attended
  • Plan of work outlining all courses in progress or planned for the Winter and Spring/Summer Semesters
  • Two personal recommendations (print or copy forms and distribute as needed)
  • If qualified you will receive notification regarding a required interview

 
Non US Citizen applicants must also include:

  • Copy of Educational Credential Evaluation (ECE) or World Education Service (WES)
  • Copy of Test of English as a Foreign Language (TOEFL)
